There could be a couple of reasons for a lost post. The most obvious would be that someone posting something would overlook the spellcheck screen, forgetting to post the message from there. Mostly, that only affects trolls and spamscripts, though.
If a post shows up in replies but not on the board, that's another symptom of the board, but partly my fault. Especially when the server is getting slammed, or a user is moving a lot of bandwidth, the message can actually post long before the script is done telling you to wait; hitting Post the sucker at that point actually leads to a doublepost. When I catch a doublepost, I download the board, remove that post, and upload the index.html again. So, if someone is posting during the thirty seconds I'm doing that, the board I'm uploading, which hasn't got the new message on it, will overwrite the newest one on the server. It doesn't happen often, but it's possible.
