.tv is still very much a country code. Tuvalu have allowed it to be used for other purposes, but that doesn't change what it is.

Regarding .us: you can easily get x.state.us, regardless where you are. The .us code is open now; whether you can get www.co.us might be in question, but, if you could, you could set up whatever.co.us as easily as setting up whatever.grem.tv or whatever.gremlin.net.

The 'net has changed a lot in the last few years. While .edu, .gov, and .mil are still regulated all to hell, you can get a .net, .org, .co.uk, or whatever, regardless where or who you are.
